Is Coffee on an Empty Stomach Bad?
For most people a morning black coffee before food is fine. Here's what the evidence says about acid, cortisol and jitters - and who should eat first.

The claim that morning coffee before food "spikes cortisol" and "wrecks your gut" gets repeated a lot online. The evidence is thinner than the confidence. For most healthy people, a black coffee before breakfast is fine. This isn't medical advice, but here's the honest picture.
The Cortisol Claim
Cortisol naturally peaks 30-45 minutes after you wake. The idea that coffee on top of that peak is harmful, or that you should wait 90 minutes, comes from small studies on caffeine and cortisol - and the cortisol response to caffeine shrinks fast as you build tolerance. For a habitual coffee drinker, the effect is minor. There's no good evidence it causes any lasting harm.
The Stomach Claim
Coffee does stimulate stomach acid and gut motility. For most people that's harmless (and the motility is why coffee "gets things moving"). But if you have:
- Acid reflux or GERD
- Gastritis or an ulcer
- A sensitive stomach that gets queasy or shaky on empty-stomach caffeine
...then eating something first genuinely helps. Otherwise it's preference.
Sensible Approach
- No symptoms? Drink your morning coffee whenever you like.
- Jittery or nauseous on an empty stomach? Have it with or just after breakfast.
- Reflux? Avoid coffee on an empty stomach, choose a lower-acid roast, and don't lie down after.
